Your pet’s safety and comfort are our top priorities. This consent form collects important health information and confirms your authorization for anesthesia and surgery. Please review it carefully and reach out if you have any questions before your pet’s procedure.

Dear Client,

Your pet is scheduled for an upcoming surgery. To help make the process as easy and stress-free as possible, we’ve put together a few simple forms for you to review and sign in advance. You will find these documents available on our website.

Please review all forms carefully. If you have any questions, you are welcome to call or email the hospital at (772) 562-7845 or ssvhreception@gmail.com. On the day of your pet’s surgery, you will be asked to review and sign an Authorization/Estimate Form. We will also confirm the best phone number to reach you during your pet’s procedure.

The night before surgery, please do not offer any food or treats after 10:00 PM. Water is fine and may be left out overnight. If your pet takes any medications, vitamins, or injections, please withhold the morning doses unless the doctor has instructed otherwise.

At your pet’s scheduled surgery time, the doctor will answer any remaining questions and collect your completed Authorization Form.

If you have elected to proceed with any recommended blood testing, these samples will be collected prior to surgery unless they have already been done. If any concerns arise, the doctor may contact you using the phone number provided on the Authorization Form.

When your pet’s surgery is complete, the doctor will go over all discharge instructions with you, process payment, and provide a written copy of the home-care instructions. If anything is unclear, please don’t hesitate to ask for clarification at any time.
